Our Passport Office is now open at our Modesto Maidique Campus, conveniently located in the PG2 Blue Garage, 1st Floor. Whether you’re applying for your first U.S. passport or renewing an existing one, our office provides fast and reliable services for students, faculty, staff, and the community—all without leaving campus.

How can Passport Services @ FIU help you?

Our Passport Office has been designated a Passport Acceptance Facility by the U.S. Department of State. Students, faculty, staff, and members of the community are able to apply for a passport directly through our office.

Passport forms can be completed in person or online at http://www.travel.state.gov and then brought to our Passport Office for processing.

You can renew your adult passport online if you qualify. Please visit mytravel.state.gov for more information and to apply online.

If you do not have access to a printer, you can print your completed form at our office. You will need to save it to a flash drive or email it to yourself to print in-store. (Fees may apply.)

We can provide photo services for your passport, visa, and other application needs.

Passport Photo: $15.00 (plus tax)

Application Fees

Application fees to the U.S. Department of State (DOS) can only be done in check, money order, or cashier’s check.

Note: We highly recommend scheduling an appointment to avoid potential long wait times. We will turn away Walk-ins if we are at capacity.

Service Fees

In-office copy of forms:

  • Black & White: $0.12
  • Color: $0.50

In-office Printing:

  • Shipping Label: $20.00 (DS-82)
  • Processing Fee: $35.00

For payments made to the Passport Office, like the execution fee, photocopies & photo service, we accept MasterCard, Amex, Discover, Visa & Debit Card.

Required Documents for New Applicants

Form DS-11

Application for a U.S. Passport completed either printed or in black ink.

Evidence of U.S. Citizenship

Examples: Original or certified copy birth certificate, original naturalization certificate, or previous passport.

Government Issued Photo Identification

Examples: Valid U.S. Driver’s License, Military I.D.) A second form of I.D. may be required for non-Florida licenses.

Execution Fee

A separate execution fee will be paid in-store and can be paid with credit/debit card only, no cash payments or Apple/Google Pay.

One Passport Photo

You may bring your own. We also have photo services available at our office.

Specifications

These documents are required for a first-time applicant who is not a minor applying for a U.S. passport in-person. For current details about requirements for applicants in other situations (ie: minors, for passport renewals), please visit Travel.state.gov.

To be seen by an Agent or be added to our wait list, all parties must be present with all the required documents and payments.

Status Tracking

Our Passport Office cannot track the status of any Passport applications. Use the Online Passport Status System to check your application status. You may not be able to get a status update for 4 weeks after you apply or renew.

Choose the option that applies to your situation below and follow the instructions provided by the U.S. Department of State.

Adult Passport Renewals

You can renew your adult passport online if you qualify. Please visit mytravel.state.gov for more information and to apply online.

If you do not qualify for online renewal, you can renew your passport by mail if you meet certain requirements.  If requirements are met, a visit to The SPOT  is not necessary and can be done at any USPS Location. Visit travel.state.gov for mailing instructions.

If you do not meet the renewal criteria, you must apply for a new passport in person.

Passports for Minors (Under 16 years of age)

All Minor applicants, including renewals, must apply in person.

Both parents or guardians should appear with the child. If both parents/guardians cannot be present, see the parental consent section for additional document requirements.

TSA PreCheck®

Now it’s even easier to enroll in TSA PreCheck® through CLEAR. Pre-enroll online and then head to our FIU Passport Office with your U.S. passport or other accepted form of ID. Enrollment is quick and easy. You can also stop by FIU Passport Office today to enroll, no appointment necessary.

Parking

Students and members of the FIU Community with a valid parking pass can park in their dedicated parking zones in PG2.

Other members of the community can park in the metered parking area on the first floor of PG2 and walk to the west side of the building to find our office.
